This was a manually entered page in an attempt to create a template for responses that would be created by PHP script written for media which was the first attempt to create a feedback / comments form. That PHP script currently attempts to send email and creates a JSON file but was also meant to add comments to a wiki page. Current replacement for this is used of modpage plugin which adds wiki text at the comments page bottom
